Instructor Course Waiver & Acknowledgement of Risk
Definitions:
1. Assumption of Risk
I understand that participation in martial arts training involves physical activity and carries inherent risks, including but not limited to slips, falls, strains, sprains, or accidental contact. I voluntarily choose to participate in the Program Level 1 Instructor’s Course with full knowledge of these risks.
2. Medical Fitness
I confirm that I am in good physical health and capable of taking part in the training. I will inform the course provider of any relevant medical conditions before participation.
3. Release of Liability
I agree that the Company, its directors, instructors, employees, and representatives shall not be held liable for any injury, loss, or damage arising from my participation in the course, except where caused by negligence that cannot be excluded by law.
4. Responsibility After Certification
I understand that successful completion of this course qualifies me as a Qualified Instructor of the Program.
I accept that the delivery of classes to children or others using the program will be solely my responsibility.
I acknowledge that I must enter into a separate Instructor Licence Agreement with the Company before I am authorised to deliver the program as a Registered Instructor.
5. Acknowledgement
I have read and understood this waiver. I accept the risks involved and agree to release the Company from liability as outlined above.
Assistant / Non-Certified Participant Waiver & Consent
Definitions:
1. Assumption of Risk
I understand that participation in martial arts training involves physical activity and carries inherent risks, including but not limited to slips, falls, strains, sprains, or accidental contact. I agree to take part in the training with full awareness of these risks.
2. Role and Limitations
I acknowledge that:
As a non-black belt or participant aged 13–17, my role is limited to learning how to assist a qualified instructor.
I will not be certified as a Program Instructor and will not be licensed to deliver the program independently.
I may only assist under the supervision and responsibility of a qualified instructor.
3. Medical Fitness
I confirm that I am in good health and capable of taking part in the training. Any relevant medical conditions have been disclosed to the course provider.
4. Release of Liability
I agree that the Company, its directors, instructors, employees, and representatives shall not be held liable for any injury, loss, or damage arising from my participation in the course, except where caused by negligence that cannot be excluded by law.
5. Acknowledgement by Participant
I have read and understood this waiver and agree to the above conditions.
6. Additional Parental/Guardian Consent (for participants under 18)
I, the undersigned, am the parent/guardian of the above participant.
I consent to their participation in the Program Level 1 Instructor’s Course.
I acknowledge the risks involved and confirm that they are in suitable health to take part.
I understand they will not be certified or licensed to deliver the program, and may only assist under supervision of a qualified instructor.
I agree that the Company shall not be held liable for injury, loss, or damage except where caused by negligence that cannot be excluded by law.
